FOMAT

Embedded sites, aligned Phase II to III

Phase II to III studies are delivered through an embedded model that connects community based medical practices, EMR based pre screening and centralized operations under one coordinated structure.

Embedded model in practice

Research staff based inside partner medical practices
Central teams for contracts, budgets, regulatory and finance
Large EMR based database of active patients across these practices
Digital systems for eSource, dashboards and real time study oversight

Phase II to III at a glance

Aligned operations

Outpatient research sites and dedicated recruitment locations in California work under one coordinated clinical, regulatory, quality, and operations structure, supporting consistent execution and representative participation with 76.88% diverse enrollment across all studies.

Faster activation

Because research is already embedded in existing practices and start up follows a single playbook, studies move from award to first patient on shorter timelines, including our 6 day record today companywide.

Engaged participants

Participants are identified through EMR, physician referrals, and local outreach, and they often hear about studies from physicians they already know, with follow up in their preferred language. 91.30% enroll internally overall.

Quality built in

Sites are ready for eSource, eConsent, eRegulatory, and remote monitoring, while teams review activity throughout study. We complete 100% QC and 5 patients’ first 5 visits within 48 hours, supporting 93.48% retention.

Early phase and Phase I unit

Alongside outpatient Phase II to III studies, FOMAT also operates a dedicated early phase unit for first-in-human studies, intensive safety monitoring, and complex PK and PD work.
