The research, a participant data meta analysis of 25 randomized controlled trials including more than 11,000 participants, has been published in The BMJ — one of the most respected medical journals in the world.

“Most people understand that vitamin D is critical for bone and muscle health,” said Carlos Camargo of the Department of Emergency Medicine at Massachusetts General Hospital, the study’s senior author. “Our analysis has also found that it helps the body fight acute respiratory infection, which is responsible for millions of deaths globally each year.”

Why Previous Research on Vitamin D Supplementation Was Inconclusive

Several observational studies have associated low vitamin D levels with greater susceptibility to acute respiratory infections. A number of clinical trials have been conducted to investigate the protective ability of vitamin D supplementation, but while some found a protective effect, others did not. Meta analyses of these trials also produced conflicting results — until now.

To resolve these discrepancies, the research team — led by Adrian Martineau from Queen Mary University of London — conducted an individual participant data meta analysis of trials in more than a dozen countries, including the U.S., Canada, and the U.K.

While traditional meta analyses compare average data from all participants in each study, individual participant data meta analysis separates out data from each individual participant, producing a higher resolution analysis that reveals patterns traditional approaches miss.

Key Findings on Vitamin D Supplementation

The investigators found that daily or weekly vitamin D supplementation had the greatest benefit for individuals with the most significant vitamin D deficiency — those with blood levels below 10 mg/dl — cutting their risk of respiratory infection in half. All participants experienced some beneficial effects from regular vitamin D supplementation, regardless of their baseline deficiency level.

However, administering occasional high doses of vitamin D did not produce significant benefits. The protective effect was specifically linked to regular, consistent supplementation rather than periodic large doses.

Public Health Implications

“Acute respiratory infections are responsible for millions of emergency department visits in the United States,” said Camargo, who is also a professor of emergency medicine at Harvard Medical School. “These results could have a major impact on our health system and also support efforts to fortify foods with vitamin D, especially in populations with high levels of vitamin D deficiency.”

The findings carry particular significance for vulnerable populations including the elderly, individuals with chronic conditions, and communities with limited sun exposure, where vitamin D deficiency tends to be most prevalent.

What This Means for Clinical Research

This study represents a high quality example of individual participant data meta analysis — one of the most rigorous methods available in clinical research. It demonstrates how aggregating data from multiple randomized controlled trials, when done with sufficient methodological rigor, can resolve conflicting findings and produce actionable public health guidance.

Understanding how clinical trials like the 25 included in this meta analysis are designed and conducted is essential context for interpreting research like this.
